Important Note to Parents and Students
Indiana State University's Student Health Insurance Plan has TWO plan options to choose from. Indiana State University recommends that both Option 1 and 2 are purchased together. Option 1 and 2 may be purchased separately but for complete protection the University highly encourages the enrollment in both Option 1 and 2. Option 1 provides $500.00 in benefits per policy year. Option 2 provides $250,000 in benefits per condition but is subject to a $500.00 deductible.
Students in needs of medical treatment will be referred by Indiana State University to the UAP Clinic located adjacent to the University. If a student obtains medical treatment from UAP Clinic or any other medical facility the student is responsible for all incurred medical expenses.
Pre-existing conditions are not covered unless you have been continuously insured for 12 months under this plan or a prior plan. Please be sure to enroll in the Indiana State University Student Health Insurance Plan before you are sick. For a detailed overview of Option 1 and 2, please view the student insurance brochure.
